Florida Health Care Plans is committed to the health and well-being of our members.
Health and Wellness programs are designed with an emphasis on preventive care to promote a better quality of life. Programs are offered using different methods to educate and inform, such as: classroom instruction, monthly mailers, telephonic monitoring and newsletters, in an effort to teach and assist our members to self-manage.
Programs are also offered for more common chronic and disease specific conditions, such as: diabetes, heart disease and high blood pressure.

Medical History Form
Health and wellness are a key concept built on good and complete information shared between you and your physician and their clinical staff. A Medical History Form, completed by you before you arrive at your physician' office, makes the best use of your time and provides your care givers with a full picture of your present illness, your general health, allergies and medication and past medical history. We may already have some of this information in your electronic health record, and if we do, we use your Medical History Form information to validate and update your important history as needed. This simple process allows your visit time with the physician to be more effective discussion rather than a one-way collection of data.
Florida Health Care Plans is your medical home - we want to know as much about you as possible so we can work together with you to identify your health care goals, identify treatment choicse with you and provide quality and cost effective interventions that give you peace of mind and motivation regarding maintaining your health!
