Complaints about Part D Prescription Drug Benefit
We encourage you to let us know right away if you have questions, concerns, or problems related to your prescription drug coverage. Please call our Member Services Department at 386-615-4022 or 1-877-615-4022 or by fax at 386-676-7149. From November 15 through March 1 the hours of operation are 7 days a week, 8 a.m. to 8 p.m. From March 2 through November 14 the hours of operation are Monday through Friday, 8 a.m. to 8 p.m.
A complaint will be handled as a grievance, coverage determination, or an appeal, depending on the subject of the complaint.
