Case Management Department

FHCP is dedicated to providing care coordination for its members throughout the system. The Case Management Department's focus is twofold, inpatient management (i.e. Discharge Planning, SNF, etc) and management of members with complex needs including Social Service needs (ie. Social Work Referrals).
