Contracting with Florida Health Care Plans

FHCP maintains a network of employed staff and community based providers to render care to our members.

Community Based Providers

Providers interested in contracting with FHCP should write to us and provide the following information to FHCPProviderRelations@fhcp.com:

  • The location of your practice/facility
  • Services you would like to provide/supply to our members
  • Medicare participation/certification status
  • Accreditation status
  • Copy of the physician's curriculum vitae (CV)

Employed Providers

FHCP employs practitioners such as physicians, therapists, ARNP's and physician assistants (PA) to work in our various facilities. All licensed independent practitioner employees work under an employment contract with FHCP.

If you are interested in becoming a full-time or part-time employed FHCP provider, please submit your CV to FHCPProviderRecruiting@fhcp.com.

Provider participation requests (Community Based or Employed) are reviewed by FHCP's Contracting Committee. The Committee bases its review on FHCP's network access and availability standards, provider location in relation to our service area, provider training, licensure status and other pertinent factors in effect at the time of review. Usually, you will receive a response back from us within thirty days of your email.
